That twelve-inch figure is worth knowing before a sign is engineered rather than after. A deep-return letter set, a raceway-mounted installation or a cabinet with a substantial projection can cross the line into a different category without the design ever changing visually — and a sign that lands in the wrong category is a sign that gets reviewed against rules it was never drawn for.

An incoming tenant often inherits sign components from the previous business, and not everything has to be discarded automatically — though a change to an existing sign is itself addressed by the code, so what looks like a simple swap still gets checked.

Generally yes. Chapter 427 requires permits for regulated signs and for new signs or changes to existing signs, and Chapter 348 may also require development approval depending on the project.
It can. Chapter 427 specifically addresses constructing, altering, enlarging or otherwise changing an existing sign, so a reface is not automatically outside the process. The exact requirements depend on the work proposed.
